Rehabilitation

Achilles Tendon Repair

(weeks 0-4)

Weight bearing: Non-weight bearing for two weeks. After two weeks partial weight bearing in boot

Brace: Patient in plantar flexion splint

No formal Physical Therapy

(weeks 4-6)

Weight bearing: Weight bearing as tolerated in walking boot with heel lift. Wean off crutches

Brace: Walking boot at all times except shower or physical therapy

Range of motion: AROM/AAROM/PROM of the ankle toe flexion/extention. No dorsiflexion past 90 degrees

Theraputic exercises:

  • Seated heel raises
  • Light resistance bands for plantar flexion /inversion/eversion
  • Proprioception exercises single leg balance
  • Straight leg raises/hip abduction/hip adduction
  • Hamstring and hip flexibility
  • Seated bike for aerobic conditioning

Modalities:

  • Soft tissue mobilization
  • Scar massage
  • Edema control

(Weeks 6-12)

Weight bearing: Full weight bearing in shoe. Wean off boot. Gait training.

Range of motion: Progressive dorsiflextion. By week 8 full ROM as tolerated.

Theraputic exercises:

  • Standing heel raises
  • Single leg eccentric lowering
  • Step ups/side steps
  • Proprioception exercises
  • Begin full lower body strengthening exercises
  • Elliptical or bike for aerobic conditioning

(month 3-6)

Progress with strengthening and proprioception exercises

Phase in impact activities and return to sport functional skills

Begin light jogging when patient can perform single leg hop on affected leg

Running/cutting and return to sports 5-6 months
